Residential Support Services
This is a group home option for individuals under the HCS program that require supervision constant due to medication or behavioral reasons. Our staff are available to respond as needed, but present 24/7.
ISS Services
Individuals in the HCS/TxHmL program can attend the provider of individualized social skills services in Texas of their choice.
Occupational Therapy
For individuals enrolled in the HCS/ TxHmL program, this service provides assistance to individuals with physical /cognitive, sensory motor, and/or physiological disabilities to assist with mastering skills that will improve self-care, work, and play.
Behavioral Supports
This service is provided to assist the individuals that are enrolled in the HCS/TxHmL program with managing their maladaptive behaviors.
Physical Therapy
This service provides assistance to individuals with physical limitations to acquire, maintain, and increase skills in the areas of mobility, strength, and dexterity for individuals enrolled in the HCS/TxHmL programs.
Community Choice
The community choice service component is the provision of assistance and support necessary for an individual who is enrolled in the HCS/TxHmL program to perform personal care, health maintenance, and independent living tasks, participate in community activities, and develop, retain, and improve community living skills.
Nursing (RN/LVN)
Our nursing services in Texas are available to individuals enrolled in the HCS/TxHmL program to monitor their healthcare status when services cannot be provided through Medicaid and/or Medicare.
Adaptive Aids
This service provides individuals who are enrolled in the HCS/TxHmL program with devices, controls, or appliances that enable individuals with disabilities to obtain or increase their skills to perform activities of daily living.
Minor Home Modifications
This service provides modification to the individual’s home to provide specialized accessibility and safety adaptations or additions, including repair and maintenance, for individuals enrolled in the HCS/TxHmL program.
Respite Care
This service is a temporary provision of assistance and support necessary for an individual to perform personal care, health maintenance, and independent living tasks; participate in community activities.
Dental Treatment
This service is provided to individuals that are enrolled in the HCS/TxHmL programs that are over the age of 21 and who are not eligible to receive dental care through Texas Health Steps or private insurance.
Speech Therapy
This service is provided to promote and increase verbal communication skills. This service assists with accomplishing goals such as progressing from gesturing to being able to communicate verbally for individuals enrolled in the HCS/TxHmL programs.
Social Work
This service is provided to assist individuals in the HCS program with coping with depression, grief, etc. This service provides counseling to assist with handling these issues.
Dietary
This service provides assistance to individuals enrolled in our HCS/TxHmL program in Texas, who have dietary needs such as nutritional, weight loss, etc. Our dietitian will assess the needs and preferences of our residents and will provide them with a dietary plan that is healthy and nutritious for them.
Audiology
This service is a temporary provision of assistance and support necessary for an individual to perform personal care, health maintenance, and independent living tasks
Supported Employment
This service provides on-going individualized support services needed to sustain paid work by the individual, including supervision and training for individuals
